WHAT IS HOSPICE CARE?
Grand Hospice is a comprehensive care service for individuals who have been diagnosed with a terminal illness and their families. We understand that end-of-life care is not just about managing physical symptoms, but also encompasses emotional, spiritual, and social support. Our goal is to provide care that is in harmony with each patient's wishes, traditions, and living will mandates.
Our team of highly skilled healthcare professionals includes a family doctor or lead physician, on-staff hospice doctor and nursing team, emotional care team consisting of a psychologist, clergy, or spiritual practitioner, nursing aides, home health assistants, volunteer companions, physical therapists, masseuse, and other body care professionals, and social worker. They work together to create a personalized care plan for each patient, taking into consideration the patient's specific needs and desires.
We understand that end-of-life transition can be a difficult and emotional time for both the patient and their family. We offer support for the family as well, through our family care team, who provide emotional and spiritual support throughout the process. Our team is available on call 24/7 to provide constant and ongoing care and support.
In terms of location of care, we provide care in the setting that is most comfortable for the patient, whether that be in their own home, our on-site palliative care facility, or at a hospital, nursing home, or other long-term care facility. We understand that each patient is unique, and our team remains vigilant to the patient's every need, ensuring that they are comfortable and respected throughout the process.
Grand Hospice services are generally covered by Medicare, Medicaid, Private insurance, HMO plans, and Managed care organizations. The procedure to establish hospice care for a terminally ill family member involves working with the primary caregiver or family team to make decisions and create a personalized care plan. Our staff will provide guidance and support throughout the process, and will monitor and assess the patient on a constant and ongoing basis.

WHAT KINDS OF INSURANCE DO YOU ACCEPT?
GRAND HOSPICE, INC. provides care on a fee-for-service basis. Generally, patients are covered by their health insurance policy or state or federal entitlement programs, such as Medicare or Medical.
Services provided by GRAND HOSPICE, INC. will be billed to the patient's approved insurance
company.

It is the responsibility of the patient or the patient's representative to disclose true and accurate insurance information to the agency. The agency will obtain all necessary authorizations from the patient's insurance company and the physician to determine home health care coverage eligibility.

HOW CAN OBTAIN MORE INFORMATION ABOUT MAKING AN ADVANCE DIRECTIVE?
Ask your doctor, nurse, social worker, or health care provider obtain more information you. You can have a lawyer write an advance directive for you, you can complete an advance directive form.
